A Physical Examination and Immunization Record is required of any new student entering Arlington Catholic High School, as well as any student trying out for an athletic team.
To meet the ACHS Athletic Requirements, it is recommended, if possible that the physical be dated after May 31.
In this way the physical will cover all three sports seasons for the entire school year. If a student continues to be involved in the Athletic Program, an updated yearly physical is required.
*** A copy of this Physical and Immunization Record must be turned into the School Nurse by August 15th.
PART A - this section should be filled out by a parent. It is imperative that we have Immunization records prior to admission
PART B - ACHS physical exam form may be used by the Physician/Practitioner or they may prefer to use their own copy of the physical exam from their practice.
Medication Permission Forms are to dispense medication during the school day. Medications that need to be taken during the course of the school day, must be accompanied by a permission note from the parent/guardian. There are certain medications on the form that may be dispensed with standing orders from our physician. If you would like to avail of this service, please check off medications on this form to be dispensed if needed during the school year and sign the permission form.

PLEASE Note any Medication Allergies on the Form.
Prescribed medications that need to be taken during the day must have the order signed by the Physician/Practitioner. A copy of this order must be on file in the Health Office is attached if needed.
A place is also designated for any Medical Conditions/Concerns and /or Medications that the Nurse, Guidance Counselor and/or Teachers should be aware of.
Please read it carefully and fill it out accordingly.
All forms should be returned to the Nurse’s Office.
